Primary Care Outreach for Seniors

The goal of this program is to improve the health of seniors and reduce the number of emergency visits and hospital admissions required by this population. Through this program, a registered nurse and community health worker are available to visit seniors in their homes to provide support with their care and link them to resources helpful to maintaining their health and independence. 

The program is free and available in English, French and other languages through the  use of interpreters. 

We accept referrals from any source for eligible seniors ages 65 and over. To inquire about this program, please call 1-844-726-5115. Referral forms are available from the link below and can be faxed to 613-739-8196.
