Pinecrest-Queensway Community Health Centre (PQCHC) and South Nepean Community Health Centre (SNCHC) are two of its service sites.
Groups are facilitated by a registered nurse and a registered dietitian who provide a variety of tools and resources that enable individuals to live well with diabetes.
Topics include:
Nutrition
Exercise
Glucose monitoring
Foot care
Self-care practices
New groups are available each month and individuals are welcome to bring a support person.
Nurses and dietitians are also available through this program for optional one-on-one appointments, in addition to the group program.
Insulin initiation is another program service provided to adults with Type 2 diabetes, who have been referred by their physicians.
